Abstract
Objective: To determine the frequency of prescription errors in a psychiatric unit in a university hospital, to check the frequency of prescribing errors by type of prescription error, and describe the most common drugs related to prescription errors. Method: Observational cross-sectional study conducted through data collection in medical records compiled in a standardized form with further descriptive analysis. Results: A frequency of 35.9% of prescription errors by prescribed medication was found; writing errors were the most frequent. Antipsychotic was the therapeutic class with higher prescription error. Approximately 69% of the drugs involved in prescription errors were prescribed for use ?if necessary? Conclusion: A high frequency of prescription errors was found, corroborating findings in the literature. Studies to assessing the causes of prescribing errors and interventions that can reduce them are still needed, particularly in the mental health area.
