Abstract
Pharmaceutical care is a practice model that guides the offer of a set of pharmaceutical services centered on the patient, which integrates health education actions, promotion of the rational use of medicines and care activities focused on therapeutic results. This practice has shown positive results in patients with chronic obstructive pulmonary disease - COPD, especially in terms of adherence to treatment and correct use of inhalers. The state of Mato Grosso proposes to offer the pharmaceutical care strategy as a public policy, in the specialized component of pharmaceutical care, adopting moderate to severe COPD as a pilot problem. The proposal is to offer a set of pharmaceutical interventions that include information, motivation and behavioral skills. This format involves basic knowledge information about the disease; measures to support their self-management and training in the proper use of inhalers. To operationalize and manage the project, didactic material (video, leaflets), action plan, a form dedicated to pharmaceutical care and a web application were created. Expected to achieve the goals of COPD treatment; improve adherence to treatment; reduce the impact of the disease on the well-being and daily routine of the patient and, in the long term, reduce hospitalizations for COPD in the state.
